An affiliate gets a unique tracking link or coupon code. When someone clicks the link, the visit is recorded against that affiliate, usually with a first-party cookie or a stored click ID. When that visitor converts, the conversion is matched back to the affiliate and the commission is calculated from your rules.
Yes. Server-side postback tracking stores a click ID and reports the conversion from your own server, so it does not depend on the browser keeping a cookie. Coupon codes and unique landing URLs also work without cookies. Most programs run a link method and a coupon method together to cover both web and offline referrals.
